Our Vision and Mission

Since 2021, the Health Proactivists Initiative has been driven by a vision to change the narrative on Africa’s growing chronic disease crisis. 

We’ve moved from awareness campaigns to action, supporting health systems, advocating for policy change, and empowering communities to promote health.


We believe that one day, every African will effortlessly prevent and manage chronic disease regardless of location or income.

We empower and support people at various stages of life to prevent and control chronic diseases and advocate for systemic change in the communities and health systems they rely on to maintain health and well-being.

By supporting people and advocating for communities and health systems that promote health and wellbeing, we prevent chronic diseases and their complications, save lives and break the vicious cycle of poverty these conditions perpetuate.

What We’ve Achieved

  • Advocated for Universal Health Coverage policies and community-level systemic reform.

  • Led the Universal Health Coverage online campaign, reaching 600,000+.

  • Supported people living with hypertension and type 2 diabetes through care and education.

  • Supported efforts to empower 300+ Nigerian families in low-income communities with COVID-19 prevention toolkits during the pandemic.

  • Built a network of 90+ passionate, predominantly youth supporters.

  • Launched 10+ public health projects reaching over 10,000 people.

  • Trained 100+ frontline community health workers on basic NCD prevention and care at the primary level.

  • Distributed essential medicines and medical devices to primary health centres across 4 states in Nigeria.
